I HEREBY certify my child enrolled is in excellent health and may participate in strenuous physical activities, including Softball, I agree to hold the Greenfield Ignitors, its servants, agents and/or employees and contractors harmless from any and all claims, demands, suits, judgments, losses or expenses of any nature whatsoever (including, without limitation, attorneys' fees, cost and disbursements), arising from or out of, or relating to, directly or indirectly, the infection of COVID-19 or any other illness or injury from her participation in this Tryout. Permission is hereby granted to the Greenfield Ignitors for my child to receive emergency medical treatment, if needed, and I certify that there are no limits to my child’s participation except as stated in writing and included in the comment in this application.
